
NASHVILLE — Tennessee Department of Correction Commissioner Frank Strada is stepping down following a catastrophic execution attempt at the Riverbend Maximum Security Institution that left death-row inmate Christa Pike unconscious and on a ventilator.
Governor Bill Lee announced Strada’s resignation and ordered an independent third-party review led by former U.S. Attorney Ed Stanton. The governor also announced a statewide pause on all executions through the remainder of 2026.
A Chronology of Failures
The botched procedure occurred when the state attempted to execute the 50-year-old inmate for the 1995 murder of Colleen Slemmer. According to witnesses and legal filings, the execution team struggled extensively, going through seven needles and spending over an hour trying to secure functional intravenous lines.
Attorneys representing Pike stated that the IV lines were improperly placed, causing lethal doses of pentobarbital to pool into surrounding tissue rather than entering her bloodstream directly. Despite surviving two full rounds of the drug, Pike remained strapped to the gurney for over an hour, showing signs of consciousness and distress before prison officials finally halted the process and rushed her to an external hospital.
Hospital staff are actively working to clear the pentobarbital from her system. She remains intubated, creating an unprecedented legal and ethical dilemma as state officials acknowledge plans to return her to the execution chamber if she recovers.
Ignored Safety Mandates
Under oath, Strada admitted that the state bypassed key preventive measures recommended by investigators following a previous near-miss in 2022, when an execution was halted at the last minute due to untested chemical components. An investigative panel had explicitly called for mandatory pharmaceutical expert oversight and strict chemical testing. When questioned about failing to hire a dedicated pharmaceutical expert, Strada defended his administration by stating it was treated as a non-binding recommendation rather than a strict requirement.
Legal advocates have heavily criticized the state’s capital punishment protocols, labeling the incident as torturous and inefficient. Prior to the incident with Pike, Strada’s tenure also faced severe scrutiny over an unsuccessful execution attempt involving Tony Carruthers in May, where medical teams similarly failed for over an hour to establish viable IV access.
Governor Lee has defended Strada’s overall service, maintaining that he operated with integrity despite the mounting operational failures. Meanwhile, legal teams continue pressing for a permanent halt to capital punishment, pointing to structural flaws that render the state unable to carry out lethal injections safely or humanely.
